Insurance

Eligibility

All full-time employees who are actively at work full-time and for full-time pay on the effective date of the policy or are on an officially approved leave of absence are eligible. All new employees become eligible for all insurance except Long-term Disability on the first day of the month coincident with or following their date of full-time employment.

Group Health Insurance Program

Pursuant to 70 O.S. § 3510(k), the Board provides continuous group health insurance coverage for all full-time employees and, at the employee’s option, eligible dependents. Employee coverage is provided at the employer’s expense and eligible dependents may be covered, at the employee’s option, at the insured employee’s expense, or payroll deduction of premiums. Dependents of eligible employees are entitled to coverage at the employee’s option and expense, subject to timely enrollment and payment of premiums.

The university shall extend the option of continuing Group Health coverage to employees and/or dependents at their own cost for a specified period of time after their date of termination in accordance with Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA).

Group Life Insurance Program

This coverage is provided at employer expense, pursuant to 70 0.S.s 351 0(k), for all full-time employees of Rogers State University. Life coverage is 2x annual salary rounded to the next one thousand dollar ($1,000) to a maximum of three hundred thousand dollars ($300,000). Salary for a summer contract is not considered in determining the amount of an employee’s insurance. Coverage is adjusted annually on the policy anniversary date for salary adjustments that occur during the year. The value of group term life insurance in excess of fifty thousand dollars ($50,000) is considered taxable compensation.

Accidental Death and Dismemberment

This coverage is provided at employer expense for all full-time employees of Rogers State University. AD&D coverage is 2x annual salary rounded to the next one thousand dollar ($1,000) to a maximum of three hundred thousand dollars ($300,000). Salary for a summer contract is not considered in determining the amount of an employee’s AD&D insurance.

Group Long-Term Disability Insurance Program

Pursuant to 70 O.S § 351 0(k), the Board of Regents of the University of Oklahoma, provide at employer expense, group long-term disability insurance coverage on all full-time employees after six full months of continuous full-time employment. The monthly benefit will be sixty percent (60%) of the insured employee’s monthly salary, not to exceed a maximum benefit of five thousand dollars ($5,000) per month. This benefit is subject to a "combined maximum limit" which is defined as: The total combined amount of benefits payable under this plan and benefits payable as the result of disability or retirement provisions of governmental and employer-sponsored plans may not exceed sixty percent (60%) of an insured employee’s monthly salary, subject to a fifty dollar ($50) minimum monthly benefit. The institutional obligations to a disabled employee are limited to the provisions of sick leave policies and the terms of the insurance agreements.

Unemployment Compensation

All full-time employees are covered by unemployment compensation insurance pursuant to 40 O.S. § 1-210(2), and at the employer’s expense.

Workers’ Compensation

Pursuant to 85 O.S. § 2b, Workers’ Compensation Insurance underwritten by Compsource Oklahoma, is provided to all employees at employer’s expense.

Governmental Tort Claims Act

Pursuant to 51 O.S. § 151 et seq., all employees are covered at state expense under the Governmental Tort Claims Act, effective October 1, 1985.
